Supported and Community Living Programs |

As part of our comprehensive services, Restore provides an option for supported long-term living for brain injured individuals who are medically stable and need the structure and support of a neurobehavioral program to live as independently as possible. The Supported Living Program allows persons challenged by brain injury to live in one of our 3 secure, home-like environments that promote community living and independence while ensuring privacy and safety. Restore promotes a healthy lifestyle for clients and establishes a personal care plan for each client. Behavior management (as indicated), social skills training, and 24-hour supervision is provided from a staff that specializes in the care and treatment of persons with brain injury with a history of moderate to severe behavior problems. Support and assistance is provided as needed for activities of daily living, to maintain each individual’s maximum level of independence.
Behaviors must be predictable and stable prior to admission to our Supported/ Community Living Programs. Initially a short admission to our Post Acute Residential Rehabilitation Program may be required to fully assess behavioral issues and develop the individualized Supported Living Program support plan. |

A minimum of 30 hours per week of combined individual and group activities are available both onsite and throughout the community to promote socialization and the highest quality of life. Community involvement is a key component of our programs. A typical client’s schedule will include not only recreational activities, but will also include volunteer and a vocational interest in community based workshops or appropriate employers in the community at large, as consistent with the individual’s ability for appropriate participation. Licensed therapy services are not provided in our Supported Living Programs. In those cases where the client might still benefit from continued maintenance levels of skilled therapies, services for appropriate treatment are available onsite at an additional cost.
Our medical director is available to act as the primary physician for the Supported/ Community Living client and the client’s personal care plan is reviewed at routine intervals appropriate to health and behavioral needs. Preventive/routine medical issues are identified and incorporated in the individual plan of care and managed by our program case managers.
Supported Living Program clients may be assigned to our rehabilitation facility site located in (Roswell, GA) or one of our two Community Living homes located in Lilburn, GA (suburban setting) or Ragland, AL (rural farm like setting). Site assignment is based on appropriate level of supervision and/or assistance needs required for personal care or program implementation. The length of stay in our Supported/ Community Living Programs is usually open-ended with options including short-term, long-term or life long arrangements.
