Our referral process is simple and designed as an opportunity to obtain an overview of the special needs of each candidate as well as to provide in depth review of how our program may be the appropriate option to meet these needs.
- Contact our Referral Relations Department 800-437-7972 ext. 8251 or referral@restorehealthgroup.com to initiate referral. You will be contacted by one of our Liaisons within 1 business day of your call.
- Our Liaison will then schedule a complimentary On-site screen conducted where the candidate is presently located, whether currently in an inpatient facility or in their own home, to determine our program ability to meet client needs.
- After review of the information collected by the Liaison, the Rehabilitation Team will determine clinical appropriateness and the Liaison will project bed availability.
- If clinical appropriateness is confirmed, the Liaison will provide a proposed plan of care to prospective funding sources as applicable and required.
- After Prior authorization for services is obtained, a transfer date is determined and coordination of the transfer is initiated with all parties.
We are happy to schedule tours of our facilities for prospective clients/families and other stakeholders.
