ADMISSION CRITERIA:
- Adults between the ages of 18-64.
- Diagnoses of traumatic brain injury (TBI), acquired brain injury (ABI, including stroke, anoxic injury), or other neurological disorders.
- Medically stable and does not need 24-hour skilled nursing care
- Ranchos level IV or above
- Requires neuro-rehabilitation; Physical therapy (PT), Occupational therapy (OT), Speech Pathology (SLP), Cognitive Retraining.
- Manifests moderate-to-severe maladaptive, neurobehavioral sequela to brain injury
- Have a viable discharge plan following treatment
Admission to Post Acute Rehabilitation Program is required prior to admission to any of our Supported Living programs. This provides a thorough evaluation of the person’s level of function, establishes individual care plans, and assures behavioral stability appropriate to the supported living setting. |
